InGenesis is seeking a Quality Management Program Coordinator to work with our client. In this role, you will facilitate the development, coordination, implementation and evaluation of clinical quality improvement activities, data accumulation, and reporting activities throughout BACH.
Duties and Responsibilities
- Acquire and maintain expert knowledge of NCQA Patient Centered Medical Home (PCMH) standards, policies, processes, and application requirements
- Responsibility for the day-to-day development and implementation of BACH s PCMH model, organization and submission of materials required by NCQA for recognition as a Patient Centered Medical Home
- Coordinate PCMH activities for the organization by:
- under guidance of Director of Compliance and Quality, develop an overall PCMH project work plan
- organizing staff and patient feedback groups
- build and lead clinic teams responsible for PCMH work plan rollout
- attend and lead meetings internally and externally regarding Quality and PCMH matters
- Collaborate with providers, staff, and leadership in order to review current clinical work flows and recommend and implement PCMH-focused changes and improvements
- Monitor PCMH compliance and develop new policies and procedures necessary for PCMH implementation while evaluating existing policies for compliance with the PCMH model
- Coordinate training activities related to PCMH data collection, PCMH factor reporting, patient tracking, policy and procedure development, and work flow changes
- Advise management of changes to continuously improve content and delivery
- Work closely with the Electronic Health Records and leadership teams relative to PCMH requirements and sustainability
- Oversee the collection, integration, and evaluation of qualitative and quantitative data related to clinical and operational performance and medical home transformation activities
- Utilize EPIC and i2iTracks data systems to create reports and review information
- Develop strategy to sustain PCMH transformation post-recognition, including sustainability plan, gaps analysis, and NCQA PCMH standards recognition upgrade
- Research and present information on programmatic tools for PCMH across the agency and the community
- Work with Clinic Managers to ensure compliance with clinical operations requirements (including work flows, day to day clinic tasks and initiatives)
- Meet with providers to review monthly KPIs, overall Clinic Measures and Peer Review data
- Oversee patient complaints and grievances process
- Perform other related duties as assigned
Minimum Qualification
- Clinical Professional with degree in Public or Healthcare Administration or equivalent combination of education and experience.
- Experience conducting program design, implementation and evaluation.
- Knowledge and experience with medical/health quality improvement methods and tools.
- Excellent communication (verbal and written) and presentation skills
- Ability to multi-task, prioritize and work under deadlines
Preferred Qualifications
- LVN, RN, Health care degree with knowledge of clinical terminology.
- Knowledge of and familiarity with NCQA PCMH requirements/application process.
- Familiarity with EPIC and i2iTracks.
- Experience working with a Federally Quality Health Center.
Physical Demands:
This position requires:
- Sitting, walking, and standing associated with a normal medical/clinical office environment.
- Regular bending and stretching; and lifting.
- Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
- Traveling to work sites including some exposure to outside elements at outdoor fairs.
- Presenting information in front of a group.
- Reading and writing in order to review records for accuracy.
- Occasional evening or weekend work may be necessary.
