In this role, you will support and assist patients across various locations and community events. As a trusted liaison between patients and healthcare providers, the CHW II will help bridge gaps in care by connecting individuals to essential services and resources.

Key Responsibilities
- Conduct outreach, engagement, and care coordination activities.
- Enroll patients in health plans using CEC certification.
- Analyze screening tool results and communicate findings with the Care Team.
- Link patients to internal and external services (e.g., food, housing, transportation).
- Participate in team meetings, case conferences, and training sessions.
- Maintain accurate and confidential documentation in compliance with agency and funder requirements.
- Support grant deliverables through assessments and data collection.
- Provide immediate crisis intervention and advocacy as needed.
- Use de-escalation techniques and program evaluation tools.
- Educate patients on health topics such as nutrition, hygiene, and substance abuse.
- Work in diverse settings including clinics, schools, mobile units, and patient homes.
- Attend workshops, trainings, and meetings as required.
- Perform additional duties as assigned by the supervisor.
Qualifications
- High school diploma or equivalent required.
- 1-2 years of experience in community-based healthcare preferred.
- Bilingual skills preferred.
- CHW Certification required.
- CEC Certification required.
- Proficiency in computer and internet-based applications.

Work Conditions
- Travel to other sites and/or locations required.
- Work outside in varying temperatures may be required.
- Prolonged periods of standing or sitting.
- Work flexible hours, which include some evenings and weekends.
