Job Description:

InGenesis is currently seeking a dedicated Community Health Worker II (CHW II) to work with our client. In this role, you will support and assist patients across various locations and community events. As a trusted liaison between patients and healthcare providers, the CHW II will help bridge gaps in care by connecting individuals to essential services and resources.

Key Responsibilities

• Conduct outreach, engagement, and care coordination activities.
• Enroll patients in health plans using CEC certification.
• Analyze screening tool results and communicate findings with the Care Team.
• Link patients to internal and external services (e.g., food, housing, transportation).
• Participate in team meetings, case conferences, and training sessions.
• Maintain accurate and confidential documentation in compliance with agency and funder requirements.
• Support grant deliverables through assessments and data collection.
• Provide immediate crisis intervention and advocacy as needed.
• Use de-escalation techniques and program evaluation tools.
• Educate patients on health topics such as nutrition, hygiene, and substance abuse.
• Work in diverse settings including clinics, schools, mobile units, and patient homes.
• Attend workshops, trainings, and meetings as required.
• Perform additional duties as assigned by the supervisor.

Qualifications

• High school diploma or equivalent required.
• 1-2 years of experience in community-based healthcare preferred.
• Bilingual skills preferred.
• CHW Certification required.
• CEC Certification required.
• Proficiency in computer and internet-based applications.

Work Conditions
