Job Description
Under the supervision of a pharmacist, performs all accountabilities associated with documenting and verifying patient home medications utilizing telemedicine.
As an integral member of the Virtual Expert Care Team, the Medication History Technician is responsible for documenting and verifying medication histories to assist providers with accurate medication lists utilizing telemedicine and virtual healthcare communication.
Skills and responsibilities for this position include but are not limited to:
Under the supervision of pharmacists, will work directly with patients and serve as one of the first points of contact for a newly admitted patient within Saint Luke's Health System.
Successful completion of Pharmacy Technician Certification Board within first year of employment.

As an integral member of the Virtual Expert Care Team, the Medication History Technician is responsible for documenting and verifying medication histories to assist providers with accurate medication lists utilizing telemedicine and virtual healthcare communication.
Skills and responsibilities for this position include but are not limited to:
- Provide complete and accurate information for each medication (drug name, dose, route, frequency, indication and date/time last dose was taken)
- Utilize available resources for medication history collection including but not limited to: patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records
- Verify and update patient allergies, adverse drug reactions and preferred pharmacy in the electronic medical record
- Correctly and completely documents updates made to the medication history in the electronic medical record
- Discusses any questions or issues that arise during the medication history process with a pharmacist
- Ability to learn electronic medical record and virtual platform
- Utilize effective listening and communication skills patients, caregivers, and healthcare professionals
- Demonstrate professionalism and communicates effectively with pharmacists, nurses, family members and others outside the hospital to gather medication history information
- Display initiative to solve problems
